OBJECTIVES/SPECIFIC AIMS: To examine maternal morbidity and its related social determinants among women experiencing homelessness during pregnancy. METHODS/STUDY POPULATION: This study will use an exploratory sequential mixed method design to explore and examine the structural, interpersonal and individual factors contributing to maternal morbidity among a convenience sample of 150 English speaking women experiencing homelessness during a pregnancy within the last 3 years in Baltimore. In the qualitative phase of the study, we will conduct semi-structured interviews with 15 women purposively sampled to refine the relationships between resilience, social determinants of health and multilevel factors that impact maternal morbidities. Factors of interest include prenatal care received, barriers and facilitators to receiving prenatal care, maternal morbidities, social support, and strategies used to manage their condition during this time. Using the findings from the qualitative phase, a quantitative survey will be developed to gather data on topics that emerged in the interviews. In addition, the Housing Instability Index will be used to measure the degree of homelessness as defined by the degree of housing instability in a 6-month period. Using the 25-item Connor-Davidson Resilience Scale, resilience levels among women in the sample will be assessed as a moderating factor in the examination of the relationship between a pregnant woman’s homeless status and maternal morbidity. Descriptive statistics and logistical regression tests will be used to analyze these relationships while controlling for other structural, interpersonal, and individual factors that may be associated with maternal morbidity. RESULTS/ANTICIPATED RESULTS: Qualitatively we expect to gain insight into the relationship between the extrinsic and intrinsic factors impacting maternal morbidities and the health behaviors and practices used by women to manage their pregnancy while homeless. These findings will inform the quantitative survey development and help generalize the quantitative findings. We expect to identify the common morbidities in this population we anticipate that there will be differences in maternal morbidity among the different types of homelessness. Maternal morbidity will be higher among women with a greater degree of homelessness. Resilience will have a moderating effect on the relationship between homelessness and maternal morbidity. DISCUSSION/SIGNIFICANCE OF IMPACT: This study, to our knowledge, is the first to look at maternal morbidity in this population. Additionally, this study seeks to move current research from examining infant outcomes at birth among mothers experiencing homelessness to understanding the maternal morbidities during this period. Long term, good maternal health has significant implications for the health of a mother’s future pregnancies and a risk reduction of adverse chronic conditions. Study results will provide the preliminary knowledge needed to guide further research leading to clinical approaches that promote better maternal health in this population. Lastly, the study findings will inform policy by characterizing the quality and strength of evidence of the adverse maternal health effects associated with the experience of homelessness.
A Case Series Evaluation of a Pilot Group Cognitive Behavioural Treatment for Children With Asthma and Anxiety

Anxiety disorders occur at an increased rate in children with asthma; however, there is only a small evidence base to support specific psychological treatments for these children. The current study evaluated the efficacy of a pilot cognitive behavioural treatment (CBT) group intervention for children with asthma and a comorbid anxiety disorder in a case series design. Five children (aged 8–11 years old) with asthma and a comorbid anxiety disorder and their mothers took part in eight 1-hour group treatment sessions. Primary outcomes measures were anxiety diagnosis and asthma-related quality of life. Secondary outcome measures were asthma symptom control and parent quality of life associated with caring for a child with asthma. Three of the participants no longer met diagnostic criteria for an anxiety disorder following treatment and three different participants reported a reliable improvement in asthma-related quality of life. Two participants reported a reliable improvement in asthma symptom control. Three mothers reported an improvement in caregiver quality of life. The findings provide preliminary proof of concept evidence for the efficacy of a CBT intervention for children with asthma and clinical anxiety.

Summary
The ability of human remains in Scotland, as elsewhere, to raise awareness of historical issues, to help us understand who we are and to teach us about race, diet, physiology, disease and the environment, is well documented (eg Pearson 1999; Alfonso and Powell 2007; Mays 2010). Investigation of the elaborate mortuary rituals that all cultures throughout history have created can also reveal to us a great deal about beliefs and about social bonding and identity (Pearson 1999; Alfonso and Powell 2007, 5-7). Perhaps most significantly, human remains have an ability to help people understand and relate to the past in ways that no other type of evidence can.
In 21st century Scotland, an average person's exposure to human remains is minimal, making death a source of endless fascination, mystery and fear. Often, exposure in a museum environment is a unique chance for some to cross these barriers, provoking a range of extreme reactions and opinions (Brooks and Rumsey 2007, 279; Roberts 2009, 33). These reactions can be important and very valuable, as archaeology and history are, at their core, about people and their stories; and it is this human connection which enables the general public to relate to the past (Richards 1999, 6). This appears to be of particular relevance in Scotland, where the significance of the heritage sector is considerable, currently attracting over 25 million visitors a year and contributing more than £800 million to the Scottish economy (Scottish Government 2011).
GAMMA RADIATION DOSES FOR PREVENTING PUPARIATION AND ADULT EMERGENCE OF RHAGOLETIS MENDAX (DIPTERA: TEPHRITIDAE)

Blueberry maggot, Rhagoletis mendax Curran, eggs and larvae infesting highbush ’Bluecrop’ and ’Elizabeth’ blueberries, Vaccinium corymbosum L. (Ericaceae), were treated with 4–1200 Gy of gamma radiation. The treatment reduced the number of immature stages that pupated and the number of adults that emerged from puparia. The lethal dose for 99.9968% mortality (LD 99.9968%) [lower and upper fiducial limits (FL)] estimated by linear regression analysis to stop pupariation was 1486 (1400–1585) Gy, at the 95% confidence level. The LD 99.99968% (lower and upper FL) estimated to stop flies emerging from puparia irradiated as immature stages was 88 (83–93) Gy, at the 95% confidence level as estimated by regression analysis. An estimated 100 762 larvae were killed, with no survivors, by irradiating 853 918 ’Bluecrop’ blueberries in bulk quantities with 71–776 Gy with a commercial irradiator. No flies or parasites emerged from puparia irradiated as larvae with ≥ 71 and > 80 Gy using commercial and research irradiators, respectively. Infestation rate of blueberry maggot larvae in nonirradiated ’Bluecrop’ and ’Elizabeth’ blueberries averaged 11.1 ± 1.2% and 14.3 ± 2.4%, respectively; the range of infestation rate was from 6.3% to 14.8% and 9.8% to 18%, respectively. Parasitism of blueberry maggot larvae by the larval–puparial parasite, Diachasmimorpha (Opius) mellea (Gahan) (Hymenoptera: Braconidae), averaged 10.1 ± 2.4% (range 3–13.3%).
MORTALITY OF MATURE THIRD-INSTAR CARIBBEAN FRUIT FLY (DIPTERA: TEPHRITIDAE) EXPOSED TO MICROWAVE ENERGY

Our investigation estimates time–temperature relationships and demonstrates the effect of rapid heating on mortality of Caribbean fruit fly, Anastrepha suspensa (Loew). Mature larvae (third instars) in water were exposed to each of seven temperatures (44, 45, 46, 47, 48, 49, and 50 °C) and each of five power levels (11, 27.5, 55, 88, and 122 W) in a research-quality microwave oven. Controls were immersed in water for 30 min and not exposed to microwave energy. Data were analyzed by a probit model with three explanatory variables. The variables were time to reach target temperature, power, and final temperature. Temperature needed to control the larvae increased as power increased. Of the power–temperature levels, the only combinations that resulted in > 99% mortality were the lowest power (16 W) at 49 or 50 °C and 30 W at 50 °C. Time required for > 99% mortality decreased with increased power. Thus, as power delivered to larvae increased and time needed to reach exposure temperature deceased, percent mortality decreased. We conclude that rapid heating imposes serious constraints on the use of heat-induced mortality; this result raises important questions that must be addressed because quarantine security may be jeopardized.
Evaluation of a general practitioner referral service for manual treatment of back and neck pain

Aim
To describe and evaluate a community-based musculoskeletal service, commissioned by National Health Services North East Essex Primary Care Trust (PCT), in terms of patient-reported outcomes and satisfaction.
BackgroundPersistent musculoskeletal conditions, including back and neck pain, are costly in terms of primary and secondary healthcare resources. Most patients are assessed and managed by general practitioners (GPs), with referral when necessary to secondary care services.
MethodPatients consulting for at least four weeks for back or neck pain were referred by their GP according to patient preference to either a chiropractor or osteopath or physiotherapist working in the independent sector. Patients completed questionnaires at baseline and at discharge from the service.
ResultsQuestionnaire data were obtained from 696 patients, 97% of whom were seen within two weeks. About half (51%) had had their pain for less than three months, and of the remainder 49% for more than 12 months. Patients received on average six treatments. Using the Bournemouth Questionnaire, the Bothersomeness scale and the Global Improvement Scale, approximately two-thirds (64.6%, 67.8% and 69.9%, respectively) reported improvement at discharge, and approximately 65% a significant reduction in medication. Almost all (99.5%) patients were satisfied with the service. Similarly, almost all (97%) patients were discharged from the service with advice on self-management; the remainder were recommended for secondary care referral.
ConclusionThis service improved patient access and choice resulting in shorter waiting times and effective outcomes. An impact analysis of the first 12 months of the service by the PCT showed a reduction in primary care consultations and in inappropriate referrals to secondary care.
